About the Role

As a Care Coordinator – Integrated Team Care (ITC) at MKAC, you will play a key role in supporting Aboriginal and Torres Strait Islander clients with chronic and complex health conditions to access coordinated, culturally safe care.

You will work closely with GPs, allied health professionals, hospitals, specialists, and community services to improve health outcomes and strengthen continuity of care.

Key responsibilities include:

  • Coordinating care for clients with chronic and complex health needs through the Integrated Team Care program
  • Developing, implementing, and monitoring client care plans in partnership with multidisciplinary teams
  • Supporting clients to access medical, allied health, specialist, and community support services
  • Advocating for culturally safe, client-centred healthcare and reducing barriers to care
  • Providing health education, care navigation, and follow-up support to clients and families
  • Assisting clients with referrals, transport coordination, and appointment scheduling where required
  • Building strong relationships with community members, healthcare providers, and partner organisations
  • Maintaining accurate clinical documentation and reporting requirements

What We’re Looking For

We would love to hear from you if you have:

  • Qualifications in Aboriginal Primary Health Care, Nursing, Community Services, or a related health field
  • Experience in care coordination, case management, chronic disease management, or community health
  • A strong understanding of culturally safe practice and Aboriginal community engagement
  • Excellent communication, advocacy, and relationship-building skills
  • Ability to work independently and collaboratively within a multidisciplinary team
  • Knowledge of the healthcare system, referral pathways, and community support services
  • Strong organisational and time management skills

A current driver’s licence and relevant clearances (Working with Children Check and National Police Clearance) are required.
