INDIGENOUS PATIENT NAVIGATOR

Salary Range: $74,100.00 to $79,797.60 annually (37.5 hours per week)

New Vacancy 

The North Bay Indigenous Hub (NBIH) will provide a wide range of programming services including traditional healing, primary care, health promotion, chronic disease management, family-focused maternal/child health care, mental wellness care, diabetes care, and a culturally integrated licensed day care facility. All of the programs are to be delivered in a culturally safe manner to the local urban Indigenous population in addition to our partner First Nations, Nipissing, Temagami and Dokis First Nations. This Indigenous Primary Health Care team seeks a motivated, self-starter who is passionate about positive health outcomes for Indigenous peoples.

Under the direction of the Director of Indigenous Primary Care the North Bay Indigenous Hub (NBIH), this position will be responsible for connecting with community partners and working with in the framework of the Models of Care to support patients access culturally competent and safe health and patient care. The successful applicant will work with community partners by providing teaching and resources to incorporate cultural safety into their services and practices.

QUALIFICATIONS:

• University Degree/College Diploma in Health, Social Sciences, Indigenous Studies
• Minimum of five (5) years of cultural learning and a demonstrated commitment to mino- pimahtisiwin
• Minimum of five years' experience working in a FNMI Community and/or working directly with First Nations Peoples.
• Ability to speak Ojibway or Cree.
• Demonstrated experience working with Indigenous culture, customs, and traditions is required.
• Knowledge of the Indigenous social determinants of health, and the context and environments in which programs and health services operate in the Nipissing Parry Sound district.
• Knowledge of systemic racism on Indigenous peoples in health and social outcomes, including specifics within the health care system, supported by significant knowledge of western and Indigenous worldviews.
• Demonstrated experience with building partnerships in the community and public speaking
• Strong understanding of Cultural Safety in Health Care
• Advocacy experience in building pathways for supportive and safe services for Indigenous community members

DUTIES INCLUDE:

• Connect with community partners to develop culturally safe pathways
• Continue the work of the Models of Care framework developed by NBIH
• Provide teaching and resources to partners, stakeholders and mainstream services to incorporate a foundation of cultural safety into their services and practices.
• Develop trusting relationships with NBIH patients to create pathways to address challenges with accessing, navigating and advocating for quality, safe health care
• Understanding of Trauma-Informed Care principles and able to apply within all relationships and interactions.
• Demonstrated ability to work as a team member.
• Demonstrate ability to problem solve.
• Demonstrated ability to incorporate Indigenous cultural values and perspectives into program designs with and for Indigenous peoples.
• Proficient computer skills particularly with Microsoft Office (Word, Excel, Outlook) and patient information systems.
• Demonstrated time management, prioritization and organizational skills.
• Must demonstrate good attendance.
• Able to work independently and engage effectively with a team.
• Demonstrated ability to balance competing priorities within compressed time frame.
• Able to be highly versatile, organized, energized and self-motivated.
• Must be able to travel between NBIH and our First Nations and community partners.

• Other related duties.
