Amity Health is seeking a dedicated Aboriginal Outreach Worker to help support Aboriginal and Torres Strait Islander people to better access and engage with available health care services.

In this role, you’ll assist clients to attend appointments, navigate the health system and maintain connection with primary and specialist services, contributing to improved uptake of care and the overall outcomes of the Integrated Team Care program.

Program Overview

The Integrated Team Care (ITC) program is funded by the WA Primary Health Alliance and delivered by Amity Health across the Great Southern, Coastal and Southern Wheatbelt regions.

The program supports Aboriginal and Torres Strait Islander people who are enrolled in chronic disease management with a General Practitioner and are living with, or at risk of developing, cancer, cardiovascular disease, chronic kidney disease, chronic respiratory disease or diabetes.

ITC provides coordinated support in partnership with General Practitioners to strengthen management plans and improve access to specialist services and approved medical aids and equipment, with the aim of improving long-term health outcomes.

About Us

For more than 25 years, Amity Health has been a trusted not-for-profit provider of accessible, community-focused health services across regional WA. We are committed to strengthening the health and wellbeing of the communities in which we live and work, delivering prevention, early intervention and health promotion services across our three core service streams:

  • Mental Health
  • Health, Chronic Disease & Ageing
  • Children, Families & Communities

Guided by our values, Accountable, Integrity, Needs-Led, Equitable, and Connected, we work closely with government agencies, health providers and community organisations to ensure people have access to quality support services.

About The Role

As a valued Aboriginal Outreach Worker, you will;

  • Provide practical assistance to support clients to attend recommended appointments, including GP or Practice Nurse consultations, specialist appointments, allied health services and Care Coordination appointments, as well as accessing prescribed medications though community pharmacies
  • Report on service activity, including attendance at initial and follow-up appointments
  • Establish links with local communities to encourage increased use (and uptake) of health services, including health checks, and identify clients who would benefit from improved access to care
  • Contribute to the effective delivery of the ITC program by maintaining accurate paper-based and electronic client records
  • Plan and prioritise daily work in response to ongoing enquiries and service demands

This is a fixed-term, part-time position, 3 days per week based in Narrogin until 30 June 2027.

About You

To be successful in this role, you must meet the following eligibility:

  • Demonstrated strong connections with, and acceptance by, the local Aboriginal community, with the ability to build trust and maintain culturally respectful relationships.
  • Highly effective verbal communication skills, with the ability to engage sensitively and confidently with clients, families and community members.
  • Sound knowledge of local Aboriginal health and community services, including the ability to navigate and refer clients appropriately.
  • Well-developed time management and organisational skills, with the ability to manage outreach activities and competing priorities
  • A current and valid C Class driver’s licence
  • A current and valid Working with Children’s Check
  • Relevant tertiary qualifications in community services, social work, or a related field are desirable; however, demonstrated lived experience relevant to the client cohort, combined with the ability to apply this insight in a professional support setting, will be highly regarded and may be considered in lieu of formal qualification
