Location – Sydney – flexible role based on where you live Permanent Full Time role – (part time 4 days may be considered) Pay rate –$47.66 per hour + superannuation  7 additional leave days per year   Up to $18,550 tax free with not for profit salary packaging Travel will be reimbursed at 0.91 cents per km in line with our travel policy  Fitness Passport discounted gym memberships  Paid Parental leave and Long Service Leave after 5 years  We have a great opportunity for an Aboriginal or Torres Strait Islander Identified Aged Care Assessor to join our Aged Care Assessment Services (‘ACAS’) team in Sydney. ACAS positions offer flexibility and autonomy and are based across both metropolitan and regional locations, with most roles involving local travel within each region.    What will your role look like?  You will be conducting assessments to identify and meet the needs of Aboriginal and Torres Strait Islander Identified clients and refer to appropriate home support services.   Day to day this includes:   Collaborating with clients, families and multidisciplinary teams as required   Working collaboratively with colleagues to promote a supportive and positive team culture   Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support services    Reviewing support plans to meet client’s changing support needs   Working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes   Maintaining currency of knowledge relating to aged care services both funded and non-funded  Assisting with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes    To be successful in this role you’ll need:  An Australian Driver’s license and access to a registered, insured vehicle   Excellent communication skills, both written and verbal  Commitment to excellence in customer service, both to internal and external stakeholders   Excellent planning, organising and problem-solving skills   Previous knowledge or work experience in the Aged Care System preferred  (but not essential) Demonstrated understanding of and to undertake person centred, goal directed, strength based holistic assessments  (or transferable skills) Demonstrated understanding of work health safety principals including the required analytical skills to identify and effectively manage risks   Willingness to apply for a Police Check  What we can offer you:  A culturally safe organisation with a Reconciliation Action Plan who is genuinely committed to reconciliation  Join a highly engaged dynamic team where you’ll be supported to grow and take on new challenges  Make a real difference in the lives of Older Australians – play a critical role in assessing their needs and ensuring they get the right care  Build valuable skills and sharpen your expertise to help you grow a career in aged care services  Flexibility and fulfillment where you can prioritise work/life balance, build strong relationships with your colleagues and make an impact on clients and their families     Aged Care Assessment Services (ACAS) operate independently of Catholic Healthcare and play a pivotal role in ensuring that Older Australians are assessed for the care and services they truly need.
