Aboriginal Support Worker (Female)

We have an exciting permanent full-time, female, Aboriginal Support Worker position based in Granville. This is an Aboriginal identified position and requires working with an Aboriginal client/s, to meet cultural and wellbeing needs.

Key Responsibilities:
- Collaborating with clients to develop individualised support plans (that take
into account their wellbeing and goals).    Â

Complete Risk Management Plans for clients

Provide support to clients to improve their independence by assisting with
living skills, transport, social activities, medication and personal care when
required.

Develop and maintain professional relationships and networks with
customers, families, caregivers and key stakeholders.
- Seek out innovative ways to enhance the clients life/journey!
- Assist in reconnection with families and Indigenous communities.
- Participating and engaging in activities and outings in the community and
building relationships and connections in the community.
- Keeping client records up to date on a daily basis using online systems by
completing detailed notes and reports on customer progress and
achievements.
You'll
also need:
- Technical literacy with proven ability to use Microsoft Office and systems
for reporting.
- Minimum Cert IV in Mental Health or Disability or equivalent industry
experience
- Availability to work flexible shifts across a rotating 24/7 roster.
- Valid NSW Drivers C Class license, with the ability to drive large cars
and vans.
- Willingness to obtain a Police Check, WWCC & NDIS Check prior to
commencing with usÂ
- Knowledge of Mental Illness (preferably working with people with mental illness) / Experienced in the disability sector with an understanding of recovery
approach's
