Aboriginal Support Worker (Female)
Permanent Full-Time Position

We have an exciting permanent full-time, female, Aboriginal Support Worker position based in Granville. This is an Aboriginal identified position and requires working with an Aboriginal client/s, to meet cultural and wellbeing needs.

Key Responsibilities:
- Collaborating with clients to develop individualised support plans (that take into account their wellbeing and goals).                       
- Complete Risk Management Plans for clients
- Provide support to clients to improve their independence by assisting with living skills, transport, social activities, medication and personal care when required.
- Develop and maintain professional relationships and networks with customers, families, caregivers and key stakeholders.
- Seek out innovative ways to enhance the clients life/journey!
- Assist in reconnection with families and Indigenous communities.
- Participating and engaging in activities and outings in the community and building relationships and connections in the community.
- Keeping client records up to date on a daily basis using online systems by completing detailed notes and reports on customer progress and achievements.

You'll also need:
- Technical literacy with proven ability to use Microsoft Office and systems for reporting.
- Minimum Cert IV in Mental Health or Disability or equivalent industry experience
- Availability to work flexible shifts across a rotating 24/7 roster.
- Valid NSW Drivers C Class license, with the ability to drive large cars and vans.
- Willingness to obtain a Police Check, WWCC & NDIS Check prior to commencing with us 
- Knowledge of Mental Illness (preferably working with people with mental illness) / Experienced in the disability sector with an understanding of recovery approach's
