Aboriginal Health Navigator 50D

The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West, Great South, Upper wheatbelt and Peel Regions of Western Australia.  SWAMS delivers a wide range of Services including: GP Clinics, Primary Health Care, Counselling, NDIS and community host various programs. SWAMS has a strong growth strategy through partnership opportunities and future community development. 

About the role 

This part time role (38 hours per fortnight – 2 days one week and 3 days the following week) is working within the Maternal Child Health team to support women and their families. This is a fixed term contract until June 2028. This role will support them during antenatal, birthing and postpartum care. The ORCHID program is a family centered program for diabetes in pregnancy.

• Coordinate and support education about antenatal and postpartum testing for diabetes and future risks for developing diabetes
• Assist ORCHID Program participants and their support people to understand and follow an individualised diabetes care plan, including any recommended health behaviour changes
• Support ORCHID Program participants’ and their support person’s use of technology to manage diabetes
• Facilitate continuity of care between services for ORCHID Program participants
• Any additional diabetes management and antenatal care barrier-reducing tasks as needed

About you?

This role will suit someone who has great people skills and who can show empathy and care for our clients. You will be supported by others in the team but also must be able to work independently.

Essential Aspects

• Aboriginal or Torres Strait Islander descent under section 50(d) Equal Opportunity Act.
• Must reside in the South West and have local knowledge about the community.
• Ability to take direction, determine priorities and manage workloads in order to meet agreed timelines and objectives.
• Demonstrated experience assisting in the development of community projects.
• Good interpersonal, written and oral communication skills.
• Demonstrated ability to work as a team member and independently with minimal supervision.
• Ability to drive to outreach to the Greater Bunbury and South West regions.
