About the Role

The caseworker is responsible to provide an effective, professional case work service including the development, facilitation & monitoring of all aspects of casework including referral, assessment, intake, case-plan, review and exit planning for vulnerable and at risk children and young people who are homeless or at risk of becoming homeless.

Primary accountabilities of the role of caseworker include, but are not limited to:

  • Undertaking holistic assessments of young people ensuring all data is captured and entered into our data management system.
  • Work in collaboration with relevant stakeholders to determine the presenting needs of the young people and the most appropriate package of support required.
  • Coordinating and monitoring support and services for young people within a therapeutic trauma informed framework.
  • Develop, facilitate & monitor all aspects of casework including referral, assessment, intake, case plan, review and exit planning.
  • Work collaboratively with significant others such as family, carers, educational staff and individual therapists to support young people.
  • Convene and facilitate case conferences with relevant stakeholders as a means of addressing the identified case plan goals.
  • Develop pathways with young people to make transitions into stable accommodation always prioritising family restoration if possible.
  • Motivate and collaborate with young people to engage them in the successful achievement of their case plan and goals.
  • Work collaboratively with clients to develop strategies and plans that addresses presenting issues, facilitates the development of new skills and perspectives, and increases engagement in education, employment, training, health/mental health and community activities.
  • Build and maintain trusting therapeutic relationships with young people and their families.
  • Undertake prevention and early intervention work through outreach to prevent escalation of issues leading to homelessness and assist clients to maintain their placement.
  • Identify strategies that support young people to develop independent living skills.
  • Drive the process of case planning and permanency planning for young people.
  • Complete assessments and collect outcome data as required for reporting and care planning.


Skills And Experiences

Qualifications:

  • Diploma or working towards a degree in social work, community services, or a related field focused on community development and engagement.
  • Additional studies or experience in trauma-informed care and casework is an advantage.
  • Aboriginal studies or experience working with Aboriginal communities is highly regarded.

Experience & Skills:

  • 2 years’ experience in case management
  • Demonstrated experience working with young people 12-24 years of age who have experienced complex trauma
  • Experience working within the Homelessness sector
  • Experience working with families towards restoration
  • High level verbal and written skills
  • Sound computer literacy skills
  • High interpersonal skills and the ability to self-reflect
  • Skills in engaging children/ young people with challenging behaviours and complex needs
  • The ability to work autonomously (under general supervision only) and to contribute to an effective team environment is necessary.
  • A sound theoretical knowledge of the youth homelessness sector and the principles and practice of case management
  • Comprehensive knowledge on working with families and presenting challenges

Work Requirement:

  • Current First Aid and CPR certificate.
  • Current Working with Children Check.
  • Clear National Criminal History Check.
  • Unrestricted NSW driver’s licence.
  • Eligibility to work in Australia.
  • Provide M180 consent to complete a verification of the Residential Care Workers Register managed by the office of the Children’s Guardian.
