BRAMS is seeking Elder Care Connectors (Aboriginal Identified) to join the team in Broome, WA. Reporting to the Community Support Services Manager, this role supports Aboriginal Elders and their families to understand, access, and navigate aged care services they are entitled to.
This is a relationship-focused role that involves walking alongside Elders, supporting them through My Aged Care processes, helping them prepare for assessments, coordinating services, and advocating for culturally safe and appropriate aged care supports.
You will work closely with Elders, families, assessors, service providers, and internal BRAMS teams to support continuity of care and positive ageing outcomes.

You’ll be someone who is approachable, community-focused, and confident building relationships with Elders. You do not need formal qualifications — BRAMS welcomes applicants with lived experience, community knowledge, and a willingness to learn on the job.

Experience in aged care, community services, disability support, My Aged Care navigation, or working within an Aboriginal Community Controlled Health Organisation will be highly regarded.
