We are looking for a committed Care Coordinator to provide culturally safe care coordination and wellbeing support to community members living with mild to moderate mental health needs. Working as part of a connected, multidisciplinary team, you will help improve access to services, build stronger referral pathways, and promote holistic health and wellbeing.

About the Role

• Coordinate culturally safe mental health care for Aboriginal and Torres Strait Islander people
• Support access to mental health services, GPs and community providers
• Deliver individual and group wellbeing education
• Strengthen collaboration between health and community services
• Build relationships with communities to support culturally appropriate care
• Travel weekly within the region to outreach locations

About You

• Identify as an Aboriginal or Torres Strait Islander person (genuine occupational qualification under s14 of the Anti-Discrimination Act 1977 NSW), you may be asked to provide proof of nationality.
• Certificate IV in Mental Health or equivalent
• Experience engaging with Aboriginal communities in a respectful, culturally informed way
• Ability to work independently and within a team
• Strong communication, relationship-building and organisational skills
• Current driver licence, WWCC/Blue Card and National Police Check
