About the role
As an Aboriginal and Torres Strait Islander Outreach Worker, this is your opportunity to engage with Aboriginal and Torres Strait Islander people who are underutilising health services to provide primary healthcare and help in accessing culturally appropriate pathways through our Alcohol and Other Drugs Program.
Reporting to the Access Health Client Services Coordinator, this part-time role offers 30.4 hours and is contracted until June 2026, located in St Kilda, VIC.
How you will make an impact
Build links with Aboriginal and Torres Strait Islander communities to promote health outcomes and encourage use of health services
Establish effective relationships with clients that support self-determination and participation
Develop individual support plans in collaboration with clients and relevant health professionals that address client needs and goals
Manage a caseload of up to 30 clients while providing information, advice and practical support across health, housing, mental health, legal and other services
Maintain accurate client records, report data monthly and identify risks to clients, staff or TSA promptly
This position is open to people of Aboriginal and/or Torres Strait Islander descent only as TSA considers Aboriginal and/or a Torres Strait Islander people are under-represented in this industry as per section 12 of the Equal Opportunity Act 2010 (Victoria)
What you will bring
Degree qualification in nursing, Aboriginal and Torres Strait Islander health, social work or related field. Equivalent knowledge skills and experience (desired)
Experience working with Aboriginal and/or Torres Strait Islander population and people with complex needs is required.
A current VIC Drivers Licence and employee Working with Children’s Check
